Would appreciate help in interpreting the following church records of Gloria Dei Luthern Church's (South Bend, Indiana) Members Born, it says:

Date of birth: May 14, 1860 (the 14 is clear, but conflicts with the Flyttningsbevis which says 04)
Received: 1892 (I assume recv'd in the Church?)
Arrived in this Place: 1888
Married: July 08, 1887 (the "7" was interpreted)
Died: no entry

Name: Anna Maria "f" (last name verified by a paper called Flyttningsbevis) - listed below.
Place or Parish: Tisselskogs (Sweden)
Date of birth: February 01, 1864 (agrees with the Flyttningsbevis)
Received: 1892
Arrived in this Place: 1888

Note: initials for the given and middle name.
Wife: Elin Sofia, Motola, Sweden, born: Aug 05, 1874, received 1898, arrived in America 1880, from Buffalo NY, removed and dismissed the same as N. J. Forsberg, her husband, above.

There is no Flyttningsbevis for N.J. Forsberg.

I am trying to understand the naming system for surnames and the locations mentioned. My Forsberg uncle is a direct descendent to Nils Forsberg. Truly would appreciate help. Nadine (
