There is a book called "Padre Pio: The True Story" by C. Bernard Ruffin. The following family tree is in that book.
1. Francesco Forgione II (Padre Pio of Pietrelcina) - born at Pietrelcina (Benevento), Italy, May 25, 1887; died at San Giovanni Rotondo (Foggia), Italy, on September 23, 1968, at age 81.
2. Grazio Mario ("Orazio")Forgione (Padre Pio's father) - born at Pietrelcina (Benevento), Italy, on October 22, 1860; married at Pietrelcina June 8, 1881, age 20; died at San Giovanni Rotondo (Foggia), Italy, October 8, 1946; age 86.
3. Maria Giuseppa ("Beppa") De Nunzio (Padre Pio's mother) - born at Pietrelcina, Kingdom of Naples, March 28, 1859; died San Giovanni Rotondo (Foggia), Italy, January 3, 1929, age 69.
Children of Grazio Forgione and Giuseppa De Nunzio:
A. Michele Forgione II - born Pietrelcina June 25, 1882; married at Pietrelcina March 12, 1908 to Giuseppa Cardone (1884-1946);died San Giovanni Rotondo Mary 9, 1967; age 85. Michele and Giuseppa had 8 children, all died at birth on early in infancy, except Francesco Forgione III (who was born at Pietrelcina on March 18, 1909 and died at Pietrelcina on November 9, 1920, of meningitis, at age 11) and Pia Forgione(born Pietrelcina January 6, 1924, married at San Giovanni Rotondo in 1945 to Mario Pennelli; the had eight children.
B. Francesco Forgione I - born Pietrelcina February 12, 1884; died Pietrelcina March 3, 1884; age three weeks.
C. Amalia Forgione - born at Pietrelcina on May 20, 1885; died at Pietrelcina February 1887; age 21 months.
D. Francesco Forgione II (Padre Pio) see above.
E. Felicita Forgione - born at Pietrelcina on September 15, 1889; married on October 20, 1910 to Vincenzo Masone (1886-1941); died at Pietrelcina September 25, 1918, of the Spanish flu, complicated by a miscarriage; age 29. Feliocita and Vincenzo ahd the following children: Giuseppa Masone (who was born at Pietrelcina on February 4, 1912, died at Arco di Trento December 18, 1930 of tuberculosis; age 18); Pellegrino Masone (born Pietrelcina July 25, 1914, died Pietrelcina September 22, 1918, of the Spanish flu; age 4); Ettore Masone (born Pietrelcina December 1, 1916; married Mariuccia Ghislieri at Loreto 1961; had one son, Pio Masone, born 1963; died at Benevento around 1978; age 62).
F. Pellegrina Forgione - born at Pietrelcina on March 15, 1892; married July 26, 1913, to Antonino Masone; died at Chieti February 19, 1944, of injuried received in a World War II bombing raid; age nearly 52. Pellegrina Forgione and Antonino had one child: Maria Giuseppa (died at age 3-1/2 on March 2, 1917); Alredo (who died in the influenza epidemic at age 14 months October 12, 1918) was allegedly illegitimate.
G. Grazia Forgione (Sister Pia of Our Lady of Sorrows) - born Pietrelcina Dec. 26, 1894; died in Rome April 30, 1969; age 74.
Mario Forgione - purportedly born Pietrelcina March 24, 1899, died Pietrelcina February 1900; age about 11 months. (Since no records have been uncovered, some doubt his existence).
4. Michele Forgione I (father of Grazio Forgione, grandfather of Padre Pio) - born at Pietrelcina, Kingdom of Naples, 1819; married twice, the second time to Felicita D'Andrea, at Pietrelcina on Sept. 23, 1859; died at Pietrelcina, August 29, 1867; age 48.
5. Felicita D'Andrea (mother of Grazio Forgione, grandmother of Padre Pio) - born about 1839; died Pietrelcina Oct. 21, 1887; age 48.
Michele and Felicita had the following children: Grazio Maria Forgione (born Oct. 22, 1860; died Oct. 8, 1946; Orsola Forgione (born Pietrelcina Jan. 5, 1865; married April 13, 1883 to Crisanto Scocca (1860-1928), with whom she had 3 children; died June 6, 1938; age 73).
6. Fortunato De Nunzio (father of Giuseppa De Nunzio, grandfather of Padre Pio) - born around 1821; married Pietrelcina April 28, 1853 to Maria Giovanna Gagliardi; died at Pietrelcina on Aug. 22, 1892; age 75.
7. Maria Giovanna Gagliardi (mother of Giuseppa De Nunzio, grandmother of Padre Pio) - born 1831; died Pietrelcina October 8, 1908; age 77.
Fortunato and Maria had one child: Maria Giuseppa De Nunzio (born March 28, 1859; died January 3, 1929; age 70.
8.Donato Forgione (father of Michele Forgione I, grandfather of Grazio Forgione, great-grandfather of Padre Pio) - born around 1786; died at Pietrelcina October 13, 1841; age 55.
9. Orsola Terone (mother of Michele Forgione I, grandmother of Grazio Forgione, great-grandmother of Padre Pio) - birth and death dates unknown.
10. Tommaso D'Andrea (father of Felicita D'Andrea, grandfather of Grazio Forgione, great-grandfather of Padre Pio).
11. Pellegrina Cardone (mother of Felicita D'Andrea, grandmother of Grazio Forgione, great-grandmother of Padre Pio).
12. Domenico De Nunzio (father of Furtunato De Nunzio, grandfather of Giuseppa De Nunzio, great-grandfather of Padre Pio).

32. Cosimo Forgione (father of Antonio Forgione, grandfather of Donato Forgione, great-grandfather of Michele Forgione I, second great-grandfather of Orazio Forgione, third great-grandfather of Padre Pio) - born around 1730.
33. Teresa Farnese (mother of Antonio Forgione, grandmother of Donato Forgione, great-grandmother of Michele Forgione I, seond great-grandmother of Orazio Forgione, third great-grandmother of Padre Pio).
Hopefully, I copied this all correctly. I am still trying to find my family's connection to Padre Pio. My uncle was very proud of the fact that we were related. Unfortunately, I didn't start our family tree until after he passed away and he seems to be the only one with any info.
