Case Pages, non-header type

QUESTION: I'm seeing pages that don't seem to fit into any category. According to the instructions, the case pages include "header area that includes the subject of the claim (in all caps), the claimant's name, and an address ..." I have five pages that are obviously the continuation of a case started on a prior page. The claimant is not mentioned by name although many witnesses are. How should these pages be keyed?

ANSWER: These pages are also classed as case pages but only the page number is keyed.
