Several examples refer to wives as Given Name, daughter of Mr Surname. In this instance do we leave the surname blank? Or assume the wife took the surname of her father and enter this instead?

Where an article continues into the second image in a set, should information be collated on the initial entry (on the first image)? Or the second page keyed as if it were standalone?
