1. When using Consecutive Index Card Form 3, be sure to key the word "Back"
2. Please key both city and state in location fields
3. Watch for death information added by hand at bottom of card

QUESTION: The card says Bell, Jessie N, see Wallace, jessie N (Mrs. Dell Wallace). Do I key all 3 names on 3 separate lines, or only use the first one?
