Questions to ask family members about deceased relatives:

1. What was the date of death?

2. What was the cause of death?

3. What was my ancestor’s age at the onset of the disease, and how long did it last?

4. Did my ancestor have any other illnesses or surgeries?

5. Did my ancestor smoke? How many cigarettes daily, and for how many years?

6. Did my ancestor drink alcohol or use drugs?

7. Did my ancestor have any miscarriages or stillbirths?
